Reporting to the Director of Care or designate, and working as part of a multi-disciplinary team, the Resident Care Manager will work closely with front line team members to provide high quality care and services to residents.  Under the general supervision of the Director of Care, this team member is responsible for assessing, coordinating, supervising, administering, and evaluating resident care and services.

Responsibilities:
  • Provides leadership, support, guidance and mentorship to the care team.
  • Supervises the implementation of daily routines, tasks and program as planned, reports unresolved concerns to the Director of Care.
  • Provides input on performance issues, such as Performance Correction Notices, Performance Evaluations, and improvement strategies to the Director of Care.
  • Incorporates the nursing process in the assessment, planning, implementation and evaluation of resident care and services.
  • Identifies the resident health care needs by collecting and documenting relevant health information.
  • Identifies priorities for nursing action, resident goals and measurable outcomes in each individual resident care plan and communicates these to the interdisciplinary team.
  • Provides initial and ongoing physical and psychological assessment of residents, reporting any relevant changes to the Director of Care.
  • Formulates the resident goals/outcomes in collaboration with the multi disciplinary team, the resident and his/her family.
  • Ensures all electronic assessment tools are completed, reviews the outcome data.
  • Verify and maintain all clinical staff are working within their scope of practice.
  • Develops interdisciplinary care plan and other case management tools by participating in meetings; coordinating information and care requirements with other care providers; resolving issues that could affect smooth care progression; fostering peer support; providing education to others regarding the case management process.
  • Confirm all resident health care records documentation is complete and accurate according to Optima Living policies and Health Authority standards.
  • Works in collaboration with the resident, family members/caregivers and multidisciplinary team to manage identified risk.
  • Utilizes and reviews risk management agreements. Communicates problems and finds solutions for improvement, reporting potential risks to the Director of Care.
  • Investigates and reports unusual incidents.  Provides corrective actions promptly.
  • Reviews key performance indicators, quality reports and inspection reports with the Director of Care and works in collaboration in the development of quality improvement action plans.
  • Actively participates in the continuous quality improvement process.
  • Completes organizational audits and reviews results with the Director of Care for quality improvements.
 Qualifications and Experience:
  • Graduate of a recognized school of Nursing with a current registration or eligible for Registration with the CLPNA
  • Previous experience or a genuine interest in providing care and leadership in the following areas:
    -Geriatric population
    -Persons with all types of dementia
    -Young adults and elderly whom are developmentally delayed
    -Persons with mental health conditions
    -Young adults with a variety of disabilities;
  • -Young adults with neurological injury or disease process
  • Demonstrated clinical competence, organizational, interpersonal and oral/written communication skills; Strong leadership skills;
  • Proficient with or has the ability to learn software such as Point Click Care, EMAR, Gold Care; The ability to educate staff, residents and family members
  • Good physical endurance and mental health
  • Able to balance numerous demands and conflicting priorities; Compassionate and caring attitude
  • Excellent critical thinking and assessment skills; supports a respectful workplace;
  • Evidence of essential leadership, communication, education, and counseling skills; Proficiency in communication technologies (email, cell phone etc)
  • Highly organized with ability to keep accurate notes and records; Experience with health IT systems and software (i.e. Microsoft Programs);

  • Demonstrates professional, appropriate, effective, and tactful communication skills, including written, verbal and nonverbal;
  • Demonstrates a positive attitude and respectful, professional customer service;
  • Acknowledges the resident’s rights on confidentiality issues, maintains resident confidentiality at all times, and follows FOIPPA guidelines and regulations;
